Obesity Management Program Information Sessions and Support Groups

Information sessions are held twice a month and are required before scheduling a visit to see the doctor. They are designed to describe the medical and surgical services of the program.  The sessions are held on Wednesday evenings from 5:30 p.m. to 7 p.m. Please register online below or call 443-849-3779. Support Groups are also held; view a list of dates.

The Information Session is held in the East Pavilion in the Civiletti Conference Center.  The Information Session begins at 5:30 PM. (Map of GBMC Campus)

Please select a date you would like to attend and then download and print out the following forms:

• Directions to the Information Session
  
• New Patient Registration Form
  
• Patient History Form 

Please submit the completed forms along with a copy of both sides of your insurance card at the registration desk prior to the Information Session.
