The Medical Records Department at GBMC is committed to maintaining the integrity and confidentiality of patient information and supports the hospital's mission and values by providing health information for patient care, quality, statistical and educational purposes.
To obtain a copy of your medical record, complete the "Authorization for Release of Protected Health Information" in its entirety (see link below). Requests for medical records may be submitted in person, via mail, fax. Scanned copies of completed and signed requests will be accepted via e-mail. Please allow 3-5 days for record requests to be processed. Records are released to other health care providers at no charge. Charges may apply if records are for your personal use.
All Birth Certificates are issued by the Division of Vital Records. You may contact the Division of Vital Records at http://www.vsa.state.md.us or call 410-764-2954. A birth notification for newborns will be mailed to you by Vital Records within 6-8 weeks. Greater Baltimore Medical Center does not issue Birth Certificates.
Greater Baltimore Medical Center does not issue or have access to Social Security numbers. Please contact your local Social Security Office or call 1-800-772-1213. A Social Security card for newborns will be mailed to you within 6-8 weeks.
