What should I bring to my first appointment?
- Referral from Primary Care Physician
- Insurance Card(s)
- Pre-Authorization, if required by insurance company
- Mammogram or ultrasounds films and reports

Will all services be covered by my insurance?
Some services may not be covered or may not be considered necessary under your insurance plan. If your claim is rejected, you will be responsible for the balance. Please consult with your insurance company.
How can I pay for my services?
Unless otherwise arranged, co-pays and deductibles, or if you don't have insurance, the full amount, is payable at the time of your visit. We accept cash, personal check, Visa/Mastercard, and offer a variety of payment options.
Can I share my medical record with other physicians?
During your initial visit, you will be asked to sign an authorization release form. This allows us to send your medical record to others, whom you designate, and only upon your request. There is no fee to prepare records for another physician or for you personal health insurance carrier.
