Information Session - Comprehensive Obesity Management Program

Become a Patient

Prior to making your first appointment, new patients must attend a mandatory Information Session led by one of our Bariatric surgeons and Nurse Practitioner. Our sessions are designed to inform interested patients about the medical and surgical options offered through the Comprehensive Obesity Management Program. Sessions are held two Wednesdays per month from 5:30 to 7:00 p.m. in the Civiletti Conference Center located in the East Pavilion on the GBMC campus, 6701 N. Charles Street, Towson, MD 21204.

Patient Forms

Prior to attending the seminar, please fill out the Patient Registration and Patient History forms and bring the following information with you.

• Patient Registration Form
• Patient History Form
• Front and back photocopy of your medical insurance card 

Register online or call us at 443-849-3779 to attend one of the Information Seminars.

In the event of adverse weather conditions, we follow the Baltimore County Public School Closings.  If the Baltimore County Public Schools are closed, then our evening Information Sessions are cancelled. 

Information Session Dates in 2010: 

 
Wednesday Evening Information Sessions run from 5:30 - 7:30 p.m.
Saturday Information Sessions run from 10:30 a.m.  - Noon.

Information Sessions are held in the Civiletti Conference Center on the ground level of Physicians Pavilion East.
