Prior to making your first appointment, new patients must attend a mandatory Information Session led by one of our Bariatric surgeons and Nurse Practitioner. Our sessions are designed to inform interested patients about the medical and surgical options offered through the Comprehensive Obesity Management Program. Sessions are held two Wednesdays per month from 5:30 to 7:00pm in the Civiletti Conference Center located in the East Pavilion on the GBMC campus, 6701 N. Charles Street, Towson, MD 21204.
Prior to attending the seminar, please fill out the Patient Registration and Patient History forms and bring the following information with you.
