| | Medical Services Greater Geriatrics of GBMC is a group of 6 ABMS Board Certified physicians and 4 geriatric nurse practitioners that care for the frail, elderly and their families. Greater Geriatrics is committed to care for this difficult and challenging group of patients across a broad continuum of care, from home to hospital to rehab to hospice. Greater Geriatrics provides direct patient care at the GBMC ACE inpatient unit, its post-acute unit, and in 12+ surrounding facilities, including Assisted Living facilities, skilled nursing and long-term care facilities, Continuing Care Retirement Communities, and the Gilchrist Center for Hospice Care. The group has limited office based ambulatory care hours. Established patients may also be seen in the home as needed. Four of the physicians are also Certified Medical Directors, providing experienced medical direction and quality management and improvement at a number of the facilities. Three of the physicians are certified in Hospice and Palliative Medicine and provide direct patient care and medical direction to the Hospice of Baltimore as well as Palliative Medicine Services to GBMC and other facilities. Greater Geriatrics’ physicians are appointed in the Departments of Internal Medicine and Family Medicine at GBMC and participate in the education of the physicians-in-training at the GBMC Internal Medicine Residency Program. Some additional services provided by the group include: - Enhanced and Consistent Coverage by Greater Geriatrics Team: 24/7 telephone access with NP/MD; regular, scheduled on-site weekend visitation to hospital and facilities
- Expedited Initial Evaluation of New Admissions: Seen within 24 hours of admission to a facility
- On-going Quality Improvement between Greater Geriatrics and the facility; Lab and medication management; review of transfers and discharges; awareness of regulatory and survey trends with monthly review of results of any state reviews at any of the facilities
- Facilitated transitions across the spectrum of care: ACE unit at GBMC; Hospice of Baltimore and the Gilchrist Center; GBMC Sub-Acute Care Unit
- GBMC Palliative Medicine Services in the hospital, in area facilities and in a patient’s home
