The Medical Records Department at GBMC is committed to maintaining the integrity and confidentiality of patient information and supports the hospital's mission and values by providing health information for patient care, quality, statistical and educational purposes.
How do I obtain a copy of my medical records?
To obtain a copy of your medical record, complete the “Authorization for Release of Protected Health Information” form in its entirety. All requests for release of medical records must include an authorization signed by the patient. Copies of medical records are released to patients at no charge.

The Release of Information office is open Monday - Friday; 8:00 am to 4:00 pm.
Photo ID is required when picking up medical records. If records are being picked up by someone other than the patient, an authorization indicating the records are being released to the individual picking up the records is required in addition to photo ID.
How do I obtain a copy of a birth certificate?
All birth certificates are issued by the Division of Vital Records. You may visit the Division of Vital Records' website at http://www.vsa.state.md.us or call 410-764-2954. A birth notification for newborns will be mailed to you by Vital Records within 6-8 weeks. Greater Baltimore Medical Center does not issue birth certificates.

How do I obtain a Social Security number for my newborn?
Greater Baltimore Medical Center does not issue or have access to Social Security numbers. Please contact your local Social Security Office or call 1-800-772-1213. A Social Security card for newborns will be mailed to you within 6-8 weeks.
