Disability and FMLA Forms
All letters and forms related to your pregnancy (e.g., proof of pregnancy/birth, travel, vaccinations, jury duty, and membership freezes) require 5–7 business days to complete. We kindly ask that you submit your requests as early as possible. Forms cannot be completed on a “while you wait” basis.

We cannot fax or submit forms on your behalf.

Typically, a return-to-work date of six (6) weeks post-delivery for a vaginal birth, or eight (8) weeks for a Cesarean section, is provided. If more time is needed, your physician will advise accordingly.
