  • If possible, keep a record of all the foods and beverages you eat and drink for 2 to 3 days prior to your visit. Include a description and amounts. If you prefer to use our form to record this information, please click on the link below to download and print the form.
  • If you have diabetes and are checking your blood glucose levels, please bring your meter and records of your blood glucose results.

- Aaron, a newly diagnosed type 1 diabetes patient with an A1c of 14.8%, arrived at the Geckle Diabetes and Nutrition Center anxious and overwhelmed, grappling with a confusing insulin regimen and a painful continuous glucose monitor (CGM) site. His CGM falsely indicated low blood sugar, prompting him to consume juice prior to the visit, yet an in-office check revealed his blood sugar was over 500 mg/dl. After physical evaluation by the educator, an infected wound at the CGM site was identified to be the culprit. Despite his hesitation to visit the ER due to a busy work schedule and multiple doctor appointments, the Geckle Diabetes Center educator emphasized the urgency, removed the sensor, educated on safe re-placement areas to accommodate his recent weight loss, and reviewed insulin dosing and carb estimation. Aaron was sent to the ER for rehydration at the end of the visit. At his follow-up, he was confidently using his CGM, administering insulin correctly, and improving with treatment for the infection. “Thank you for caring about my health,” he said, grateful for the guidance that empowered him to manage his condition effectively. In three months, he has brought his A1c down to 7.2%.

- Jane is a 74-year-old woman who has lived with diabetes for more than 10 years. When she was first seen by the Geckle Center, her illness was poorly controlled. She was a caretaker for her husband and didn't always take the time to think about her own needs. In April 2025, her A1c was 9.9%. She had tried a Dexcom G7 continuous glucose monitor in the past but was unable to learn how to use it. Her physician thought this was an important part of her diabetes care and referred her to the Geckle Center to learn how to put on the Dexcom G7, how to interpret it, and how to use the data. Her first visit with the Geckle Center was in early May 2025. At that visit, she had 19% lows and 5% very low. The goal is 5% or less lows (which are defined as less than 70). The educator reduced her basal insulin to help lessen the lows and worked closely with her on important lifestyle changes that would help to reduce her excessive lows. At her visit in early June 2025, she only had 1% lows and 68% of blood sugars were within the 70-180 range. The goal is to have greater than 70% in the target range. She was able to put the Dexcom G7 on independently. By late July, she reduced her A1c to 7.4%. The goal for most patients is to achieve an A1c of less than 7% if this can be achieved without too many lows. She now understands much more about her diabetes and her confidence level has improved tremendously. Unfortunately, her husband passed away in July, but the lessons were so well learned that her sugars only fluctuated slightly during this difficult time. When she met with the educator in early August 2025, her lows had increased a little to 5% with less than 1% very low and she had 66% in target. She knew this was due to not eating consistently and making a few poor food choices. She is aware that she has more work to do and will be following up with the Geckle Center in another month. She is grateful that she has the resources of support to continue to work on being even healthier.
