It's a yearly appointment that focuses on your overall wellness; it is not a traditional routine physical exam. Together with your GBMC Health Partners primary care provider, you'll review your health history, identify potential risk factors, and create (or update) a personalized health plan. Topics discussed during a Medicare Annual Wellness Visit often include recommended screenings, preventive services, and strategies to help you stay active and well.

How to Schedule Your Medicare Annual Wellness Visit

If this is your first year as a patient with Medicare, please contact your PCP office below to schedule your Annual Wellness Visit. Many of the PCP offices will utilize scheduling workflows at the time of your visit to ensure that future visits will occur at the appropriate time, so you may be asked to schedule your Annual Wellness Visit if you are already in the office with us for another appointment. You may also be able to utilize scheduling in MyChart directly, selecting the choice for an "Annual Physical" visit, but please call your PCP office if you need help to find an available time.

I came in for a Medicare Wellness Visit, but there ended up being a charge. What happened?

Occasionally, a patient may come in for an annual wellness visit, but during the visit, they inform their provider of a new or worsening health issue that needs additional attention that is separate from the focus of the wellness visit. When this happens, GBMC providers will prioritize your greatest needs first; in this case, we may prioritize the health issue as a visit, and, if necessary, change the annual wellness visit to another day. Charges for standard office visits that are not annual wellness visits may vary based on any Supplemental Insurance Plan or Medicare Advantage Plan that you may or may not have.
